I just added the upload modification and it seems that the form portion of it is working correctly (as far as viewing the form to enter info and select a graphic) until I click on Add Record and then I get an error. I am not sure what is wrong...

Is there someone who can take a quick look at the files who is familiar with this mod?

Note: I also added the short/long display to this before the upload mod.
