When I test the rate.html everything does what it's supposed to except when i rate something like i put it to 10 then I click the rate it button, it says this page can't be found. What do I have to modify?

The code I used for my rate forms (rate.html template) and links is below, hope this helps.

Are you sure its a page not found? Also, what URL does it show in the address bar, is it what it should be?
