When setting up for the first time, the install script comes up with:
Error: Could not connect to database. Reason: Unknown database 'ihdata'

Does this mean I have to "set-up" a database BEFORE I run the setup?

Shouldn't the setup script do this automatically.

How to I create this database?

No, the script requires a database to be setup. You can use mysqladmin to do this. See http://www.mysql.com for more information.
