Where is the maintain.cgi - is it a file in a folder? Or a subroutine in a file?

I am getting an error at editor login: it cannot find the page at all. (Looking at the data in admin, the editors were correctly created.)

I am running in static mode and my static pages are in: http://www.cyberinstitute.org/linksql/ so there is no overlap.
