I have come across a site using Links SQL Needscript.com.
They are using a Get rated script to allow user to generate their code to use on their web site:

Does anyone have any idea on how that is done?

Hi. It looks like a custom script;

It would be pretty simple to write (just let them pass in the ID number of their link, and it will then generate the HTML form for them, so they can place it on their site).
