sub html_record_form syntax error

sub html_record_form syntax error
Hello,

I would appreciate some assistance, please. I have placed a text file of a snippet of the html_record_form at:

http://viewpointdesign.net/...s/proofs/test_pl.txt

The problem is that I keep getting a syntax error at line (or above) 60. Error reads as:


syntax error at C:\SME\Client Files\View Point Designs\cgi-bin\plans/test_1.pl l
ine 60, near ")
if"
C:\SME\Client Files\View Point Designs\cgi-bin\plans/test_1.pl had compilation e
rrors.

It has something to do with this entry:

if ($per_mod)
if ($per_admin) {


but I am stumped as to the problem. I have tried several things, none of which work.

Can someone point me in the right direction?
