Probably a problem with permissions...you need to have the url.db file set to the following permissions:

Code:
IWAM/MACHINENAME = Read, Write
IUSR/MACHINENAME = Read, Write
Everyone = Read, Write
Admin = Read, Write, Execute, Delete
