We are currently running GM v2.2.3 (which I've recently installed). However, this problem existed prior to this and was the catalyst for upgrading to the latest version.
When I checked the 'bounce' directory I found 43429 message files using over 32MB of disk space. As soon as I deleted all of those files GM started processing emails.
In relation to the above when is outgoing.pl called? Should it be run as a cron job?
