Extra Null record with default add_form.html

Extra Null record with default add_form.html
MadDBMan SQL Version 2.1.0, Mysql 4.0.17, Solaris 2.8

reduced DB to 3 fields; 1 primary key autoincrement and 2 text.

When I add a record using the admin interface (admin.cgi) all is fine ( a single record added )

When I login and use the 'default' template (db.cgi) record is added and an additonal null record with the autoincrement ID is added.
