What template is being used? It should use 'login.html' for that page - you can see it has been changed by going to the Setup -> Actions section of the admin panel, and editing the 'login' action. The 'login' template should be 'login.html'. Alternatively, you may have modified the login.html page - if so, you can restore the default version it from the template editor.
