You need to install a perl module called Apache::DBI. Ask your hosting company to install it for you, or, if you have root access to your system, just type (as root) in a shell (i.e. through telnet/ssh):

perl -MCPAN -e 'install Apache::DBI'

That will install the necessary module for you.
