Hi there.

I've just started using DBManSQL and I'm trying to put together a template set for the first database I'm setting up and I've run into a heap of confusion. Basically, I'm trying to make a working "add_form" template that integrates with my site, but for some reason with what I've written, nothing seems to happen when you submit.

More specifically - I hit submit, and I am returned to another "add_form" page. The only apparent difference is that the "do=add_form&db=volunteers" is no longer in the url (presumably since it has been "posted" from the previous form).

I've attached the template in its entirety. I would be very grateful for any help or advice.
