I want to display the forms to add a record to the database
with default values for the textfields. The values are saved in the same directory in a textfile.

for example i have a form with 2 fields

field 1 name
field 2 birthday

the textfile contains:

name = Luis
bithday = 03.08.1967

each time the page for adding a record will be displayed i want that
the fields contains the values from the textfile as default

How can i do this
