1) Check to make sure you uploaded all the files in ASCII mode.
2) Check to make sure that the file permissions are set to 755 (rwxr-xr-x)

(as clearly stated in the README file)

Also, check with your hosting company to make sure that you can execute CGI/Perl scripts outside of the cgi-bin directory.
