It looks like DBD::mysql isn't installed. You'll have to do it manually. As root:

cd /root/.cpan/build/Msql-Mysql-modules-1.2215
perl Makefile.PL
make
make test
make install

and if it fails in one of those steps, post what failed and what the error message was.
