i use ur pass code but i can't add new url?

hi road runner a like your code for protecting visitor changing the url with password. But when i install it, it works only for modify but i can't add new url? it says something like "password can't leave blank" although i have insert password field (filed name is pass word) on add form? I try to read again your instruction but i have done all the instuction?

how can i fix my problem?
