I have read the information in the Forum as well as the FAQ and I am a bit confused on creating my own forms. I don't want to use the auto generate option. Do I create the HTML page or do I have to use the html.pl form and just copy and paste it into that form?

Is there a thread on this or some information with a tutorial that I have missed?

What you need to do is create your custom html and replace the html coding in html.pl under the sub html_record_form. However, you will then need to change html_record also so that each field gets printed out. Just follow the same format as what is currently in html.pl
