email authentication

Are there any painless and tested hacks or third party extensions
that make it so users have to provide a working email address in
order to create an account?

You can restrict their ability to edit before they confirm their E-mail,
by setting:

# Allow users to edit only with a validated E-mail address
$wgEmailConfirmToEdit = true;

in LocalSettings.php.

They can request a new email from their preferences.
