Previously working connection now failing

I've attached a Debug 3 trace. Connection seems to be going through the
mode config correctly as it gets in to the "S7.2 QM_packet2
send_receive" stage but the server doesn't seem to like what was sent at
this stage. Any ideas on which particular bit the server is choking on?
