Pages and Title Index Structure

Hi All,

I realise this is a simple question that is documented, but I can't
seem to wrap my head around this one. So here goes.

In a default install of trac I notice in the TitleIndex there is a
nice nested unordered list of all the pages with nesting that suggests
a parent child structure. Very Nice! Yet I can't seem to re-produce it
per how the documentation suggests.

I have tried many things mentioned in the documentation such as

create an initial page [wiki:FirstPage] or just FirstPage from the
wiki start page. Then create the page and edit it, create a new link
like [./SecondPage Second Page]. But when I view it in the index all
is see is

FirstPage
FirstPage/SecondPage

I don't get the pretty tree structure. The only way I have been able
to create a TitleIndex structure is to use an absolute path every time
I create a link. So by the time I get to a third page for example if I
want to create a 4th and have it representative of the parent child
structure I want is to do [wiki:FirstPage/SecondPage/ThirdPage/
FourthPage Fourth Page].

So that is okay I guess but the documentation suggests I shouldn't
have to do that. Am I missing something here? Is there an easier way
to create that hierarchy without have to use the full absolute path?

The TitleIndex macro leaves much to be desired. You might get some
insight from the following two tickets:
