They have to start with X-Spam- (note dash instead of underscore). What happens when you run a message through SA on the command-line? ie: spamassassin < testmsg.txt Are you using something like this exim integration: http://www.debianhelp.org/node/10614 In which case, you'll have to edit that exim script, because SA isn't generating headers in that kind of setup.
