Hi List, i have a file which contain the IP which i want to DENY from the SMTP level using "tcp.smtp"/tcpserver. The file is a continuous file. Is it possible.

Write a shell script that creates the proper lines for "tcp.smtp" and runs "qmailctl cdb". Or you could write an external program that you start from tcpserver instead of qmail-smtpd, which only starts the smtp server if there's a non-match in your blacklist file.
There's probably as many ways to do this as there are breadcrumbs in my keyboard.
