Low hanging fruit

I'm not going to do commits on my own in this stage, but there is a
small error in perlxs docs:

pod/perlxs.pod [at] 194: RETVAL = newSVpv(MY_CXT.name[index - 1]);

newSVpv () is missing the second argument

Did this message result in anything happening? I'd hate to see a real problem
fail to get addressed because it fell through the cracks during code freeze.

Actually, it's broken in other ways too.

I'll push a fix.
