[PATCH] fix B::Debug pmnext

The attached patch for blead and 5.10 removes printing the
B::PMOP::pmnext member.
B::PMOP::pmnext is long gone. Also some minor doc fixes.
The walkoptree_debug method for LISTOP is broken, I have to investigate why.
