Good card marked bad

Upon inserting a G8Ti into a running 6808, one of our G8Xi cards immediately
started logging the following messages:

Now we can't get the card to come back, it says:

12/22/2006 17:21:21.11 <Erro:SYST> Card 1 already had Hardware Failure - set
to Fail state
12/22/2006 17:21:18.16 <Info:SYST> Power up card 1
12/22/2006 17:21:18.11 <Info:SYST> Detected card[1]

Any way to clear the remaps and rescan this card, I suspect a weird software
glitch caused it to bounce like this to the point of failure, since the card
has been running with out problems for years until we tried adding a G8Ti in
another slot.
