Quick'n'dirty port of the themedmenu code to new arch.
- No transitions between menus
- No return notification from menu (back to toplevel)
- Buncha stuff if'd out
- No x/y offset (minor)

Need to refactor a bit to better fit with new arch (add items as children, not drawing internally)

The "depends" tah can now specify multiple plugins. If any one of the plugins listed is prenset then findDepends will return true and the menu item will show up. This is handy for top level menus that group plugins.

Treat each level of the menu as an indivudual screen. Probably slightly broken (I know switching themes on the fly probably is), but can be fixed later.

Make text outlines more transparent than their parent text. Looks nicer, and
fades in and out much nicer, too.

* Merge similar positionMap/seekTable code from *Decoder classes into
existing base DecoderBase class. This patch consolidates the following
functionality into the base decoder class:

DoRewind()
DoFastForward()
sync PositionMap as SyncPositionMap() and supporting methods from
avformatdecoder.cpp.
SetPositionMap()

The changes have been tested with mpeg2 files from ivtv-compatible cards
and with mpeg4/nuppel files from a bttv card. The two things that need
heavy testing to make sure things weren't broken in the consolidation are:

1) the ivtv decoder output for people using a PVR-350 for output
2) the keyFrameAjustMap code for people who have transcoded recordings
and used the cutlist to cut out portions.

We need to make sure that skipping forward/backward in files works
correctly for the above two cases. We wanted to get the code put into CVS to get it exposed to a much wider test base. This is a much better
solution for the short-term fix I put in place a couple weeks ago to
fix the seeking problems people were experiencing especially when
watching an in-progress recording.

* Add a --rebuild option to mythcommflag. Running mythcommflag with --rebuild
will rebuild the positionmap/seektable IN THE DATABASE for a recording
without going through all the motions of doing the commercial flagging.
This runs very quickly since it doesn't have to decode the video or audio,
it only scans through the recording. mythcommflag can now rebuild the
positionmap/seektable in about the same amount of time it would take to
copy the file.

NOTE: This DOES NOT fix the seektable in bad nuppel-format files, it only
saves a positionmap/seektable to the database.

Make discwatching plugin check symlinks more thoroughly when trying to
determine if device names are identical.

Make each device watcher inside discwatcher properly remove its metadata
when shutting down.
