I have AirTunes support enabled in MythTV and iTunes and installed the
RAOP key. The frontend show up in iTunes too.
But if I select the frontend as the speaker, the selection goes back to
the internal speakers. I can't permanently select the frontend. There are
no error messages in iTunes (AFAIK) or mythfrontend logs.

You need to provide more information such as mythfrontend logs (with -v
audio,playback --loglevel debug)
Where the frontend is running, what iTunes version you are using and on
which platform (PC or mac)

It certainly works for me and has been extensively tested by many users

AirPlay and AirTunes are largely reverse engineered hacks. RAOP (what you
call AirTunes) wasn't even working with iTunes until very recently...

Unless I'm provided with a macos 10.4 machine, I won't be able to debug
what's going on...

The time for me spent to track and debug what's going on is going to be
greater than the cost for you to get a newer mac.

10.4 is old and obsolete, you can't expect support to continue,, not even
Apple supports it anymore.
And if you're using an Apple TV 1st gen, even getting iTunes on it was a
hack.

You can start a bounty if you're that keen..
