Changes since 0.14:
use PERL_NO_GET_CONTEXT macro to make the module run faster under
ithreads-enabled perl
fix req_time doc (returns msec, not usec)
added a new method $image->server_limit, which is the same as
HARD_SERVER_LIMIT added for future compat with Apache::Scoreboard 2.x.
