Modified patch to MySQL RA

Hi Dejan,
here's a modified patch to the mysql agent for the commit version
4c18035. In addition to the previous changes, I made the IO thread
state check case insensitive. Stumble on that yesterday with a
customer, 5.5 uses lower case.
