#0.3.2k patches and small DMA fixes

This has the last couple patches and some small fixes to the DMA
changes, and cleanup of unused DMA code, consolidating the DMA
code even more. Also I have decided to run Lindent on the entire
ivtv source, so now everything is indented standard like the Linux
Kernel style calls for.

I am really curious how this works for those with the previous DMA
problems when using both the encoder and decoder together, or general
DMA errors.

#0.3.2k - some cleanup and fixes to last DMA changes.
- hotplug firmware no longer uses kmalloc
- remove semi colons from cx25840 driver
- run Lindent on cx25840 driver.
- run Lindent on entire source of ivtv.

Hi Chris,

Is there any chance you can post an intermediary diff between 0.3.2j and
0.3.2k which includes the functional changes, but excludes the Lindenting,
for the benefit of people following the changes to the DMA code?

Alternatively, could anyone recommend a more intelligent tool than diff -b
for trying to track changes in the code's structure as opposed to the
code's layout?

Hm, I just realised in my stupidity that a smarter solution would be for
me to simply run Lindent on a copy of 0.3.2j, and then diff away as normal
with the new prettified 0.3.2k ;)

I used to get DMA errors after < 45mins of playing out the PVR-350's
TV-out in MythTV, so here's what happened with 0.3.2j:

Out-Of-Memory errors after ~2.5 hours of constant playback.

Abit BP-6 (66Mhz bus) dual Celeron 500
384Mb of RAM, 2Gb of Swap
Vanilla Linux 2.6.11 SMP Preempt
PVR-350

Attached are my syslog with the OOM errors, and dmesg with ivtv info.
Hope it helps.

diff -bu ? Should ignore whitespace changes.
