We have the same problem over here and to mee it occurs if this has to do with
several MX hosts for a mail domain. We had some cases recently where the first
tried MX hosts returned back a 450 due to greylisting and the last two refused
the connection completely. After that the mail instantly went into "retry
timeout exceeded" and was bounced back. Could it be, that the "Connection
refused" errors for the host are adding up so that at some time it leads to
immediate bounces even if some other MX hosts only defer the mail?
