improve AUTH PLAIN docs, fixing 2 bugs
I found two bugs in the documentation where PLAIN is used peripherally, by way
of example. Fixed those. Added pwcheck example per the pam example just
above. Added a warning to the end of the section on the default configuration
file, after it explains what's present and why.
I haven't rebuilt spec.txt, but this patch passes visual inspection. ;)
