I'm seeing behaviour very like this, and rather than opening a new bug, I
thought it best to reopen this one.

I'm using debian's version 4.69-6 of exim4-daemon-heavy.

This is very much an intermittent thing, so it's going to be hard to
reproduce... however, here (reject.txt) is a message being rejected
immediately. (Email addresses have been changed to protect the
guilty^Winnocent, and this mail had a bunch of other recipients which I've also
elided from the log).

The retry database shows the following for this address:

R:xxx[at]fastmail.co.uk:<xxx[at]rvanderhoff.org.uk> -44 13133 SMTP error from
remote mail server after RCPT TO:<xxx[at]fastmail.co.uk>: host
in2.smtp.messagingengine.com [74.52.187.90]: 451 4.7.1 <xxx[at]fa
04-Mar-2009 13:51:29 09-Mar-2009 11:05:24 09-Mar-2009 17:05:24 *

Looking at the logs for 4th March, there was indeed a message which failed at
13:51:29 - but it got retried and subsequently delivered.
