Bob, Someone on the group had a similar issue and was able to use a alternate tftp server to upgrade the phones as this tftp server was local on the site.If I remeber correctly he had used solar winds free tftp server to upgrade the phones at a low bandwidth site .Search the archives on the list and you can use this method to upgrade the remote phones.
