from "sh ver":
System returned to ROM by reload at 22:23:32 EDT Wed Oct 27 2010 (SP by reload)

Last reload reason: Maintenance

No crashinfo
Nothing in "sh hist"
Nothing in "sh context"
Nothing in "remote command sh nvlog"
Nothing in "sh thec-support" that stands out.

My understanding - "Maintenance" for *Last reload reason* is user specified NOT system-specified.

What am I missing here?

Can someone (perhaps from Cisco) explain?

Looks to me like someone typed:

Router# reload reason Maintenance

If you have AAA authorization logs, you can check to see who did this.
