For the path /usr/sbin/sendmail ; do I substitute usr with my user name?

Also When I modify the Add URL form to my liking; it shows up as 404 when I hit send. Need help with this.

Nope, /usr/sbin/sendmail is the location of Sendmail on your server. Some servers have it located at /usr/bin/sendmail, others have it at /usr/lib/sendmail, and others may have it at different locations. I would reccomend looking at your Hosts FAQ pages just to check where the location of Sendmail is on your server....Hope this helps
