Your problem is not in the db.cgi file. Your problem is that you told me you wouldn't be using the signup feature and so we deleted the signup form subroutine. If you are going to want to use the signup feature, you'll need to put the subroutines (html_signup_form and html_signup_success) back into your html.pl file.
